Register for an EMT Test
 

If you took a EMT Basic Class or Core Class in the past year conducted by the EMS Academy at Underwood Memorial Hospital you can fill out the form below to request they you be registered for an EMT written test.

 

PLEASE NOTE Test requests must be submitted to OEMS for their approval at least 2 weeks prior to the test date  - However some tests will close when they have reached capacity and this may be earlier than 2 weeks before the test. We will make all attempts to register you for the date you request. No student is permitted to sit for testing without approval from OEMS.

 

ALSO NOTE - The list of students approved to attend the test is published by OEMS about 1 week prior to the test date. Plan on contacting the EMS Academy a few days prior to the test to ensure that your request to test has been approved by the State department of Health.

Valid Healthcare Provider CPR Card < no copies>

Several #2 pencils

A check or money order for $20.00  payable to NREMT <unless you qualify for the EMT Training Fund - then bring a signed training fund form with you>
